Address Change Notification Procedure

If your personal information has changed since your application, please notify the Japanese Society of Cancer Nursing Secretariat of the change by fax or e-mail as soon as possible. To download the membership information update form, click on this option.
Otherwise, include your membership number, name, gender, birthdate and details of the changes you wish to make in your registration and send the information to the secretariat:

Members wishing to cancel their membership must submit to the Secretariat a written request.
Request about personal information change and queries about membership fee payment, please also contact the Japanese Society of Cancer Nursing Secretariat by fax or e-mail.
